Getting Started

Applying for a grant requires gathering some details. Only the applicant, or the applicant’s authorized Personal Representative, may complete the application.

Here are the four steps involved:

Collect needed information.

Fill out the online application.

Submit requested documents and invoices.

Step 1: Collect

The more information you can give us using the online application, the more quickly we can process your request. We will collect any missing information during the phone interview. This is what we need:

Applicant information. Name, address, email, phone number(s).

Personal Representative information. If an applicant is not able to fill out the application, then someone authorized by the patient to act as their Personal Representative may fill out the form for them.

Care-provider information. Name, phone number, and dates of service of the Christian Science nurse or facility.
